Nurse Clinician- Fostering Connections Program & Child Assessment

Provides nursing care and coordination services for specialized pediatric and adolescent patients placed in foster or out-of-home care. Fosters effective communication among health care professionals who provide care to children with altered health states. Develops and implements educational programs for patients, parents and families, hospital staff, and community health personnel. Collaborates with partnering agencies to provide medical services designed to help foster families and kinship caregivers navigate the healthcare system for these various needs; while improving continuity of care among other providers. The program streamlines access to behavioral health services, developmental services and other providers, which enables children to receive the care they need in an efficient and timely manner. Practices professional nursing and promotes patient advocacy in accordance with the ANA Code of Ethics for Nurses. Maintains a current Ohio Nursing License.
